The FCC’s Wireline Competition Bureau has issued a Public Notice providing guidance regarding new forms for the Lifeline program that both eligible telecommunications carriers (ETCs) and consumers must use to verify and recertify subscriber eligibility effective July 1, 2018. The new forms include the following.
- FCC Form 5629 (Lifeline Program Application Form) to obtain information to evaluate whether a consumer is eligible to receive Lifeline service and obtain consumer certifications.
- FCC Form 5630 (Lifeline Program Annual Recertification Form) to recertify the eligibility of existing Lifeline subscribers.
- FCC Form 5631 (Lifeline Program Household Worksheet) to obtain information to confirm whether consumer eligibility in shared address instances..
These forms are intended for use in all states and territories, regardless of whether the National Verifier has been rolled out in that state or territory.
